We connect individuals who have experienced traumatic brain injuries (TBI) or concussions with independent, vetted specialists. CBI is not a medical provider, law firm, or insurance company. All clinical decisions are made independently by the specialists in our network.
2. Information We Collect
We collect the following categories of personal information:
2.1 Information You Provide Directly
- Contact form submissions: name, email address, phone number, user type (patient, attorney, physician, family member), subject, and message content.
- Referral form submissions: patient name, contact information, referring party information, and general nature of the injury or case (submitted through our secure referral portal).
- Lead capture forms: email address, name, and firm name (for resource downloads).
- Phone inquiries: information you share during phone calls to our referral line at (877) 780-6711.
2.2 Information Collected Automatically
- Usage data: pages visited, time on site, referral source, and interaction patterns via Google Analytics.
- Device data: browser type, operating system, screen resolution, and IP address (anonymized where feasible).

- Care coordination: To match patients with appropriate TBI specialists based on location, presentation, and clinical needs.

We do not sell, rent, or trade your personal information to third parties for marketing purposes.

- With independent specialists: When you request a referral, we share relevant contact and injury information with the matched specialist to facilitate your evaluation. Once shared, the specialist’s handling of your information is governed by their own privacy practices and HIPAA obligations.
- With referring parties: If an attorney or physician referred you, we may confirm scheduling status with the referring party.
- Service providers: We use third-party services for website hosting, analytics, and email delivery. These providers are contractually bound to protect your data.
- Legal obligations: We may disclose information when required by law, subpoena, or court order.
5. Protection of Minors (Under 18)
CBI recognizes that traumatic brain injuries affect individuals of all ages, including children and adolescents. We take the protection of minors’ information seriously.
- Parental/guardian involvement: Referrals for patients under 18 must be submitted by a parent, legal guardian, attorney, or treating physician — not by the minor themselves.
- COPPA compliance: We do not knowingly collect personal information directly from children under 13. If we become aware that we have inadvertently collected such information without verified parental consent, we will promptly delete it.
- Limited data collection: For minor referrals, we collect only the minimum information necessary to facilitate the specialist match (name, age range, nature of injury, and parent/guardian contact information).
- No marketing to minors: We do not target advertising or marketing communications to individuals under 18.
- Parental rights: Parents or legal guardians may request access to, correction of, or deletion of their child’s personal information at any time by contacting us.
6. California Privacy Rights (CCPA/CPRA)
If you are a California resident, you have the following rights under the California Consumer Privacy Act (CCPA) as amended by the California Privacy Rights Act (CPRA):
- Right to know: You may request that we disclose what personal information we have collected, used, disclosed, or sold about you in the past 12 months.
- Right to delete: You may request deletion of your personal information, subject to certain legal exceptions.
- Right to correct: You may request correction of inaccurate personal information.

CBI is a care coordination service, not a covered entity under HIPAA. However, we treat all health-related information you share with us with the highest level of care and confidentiality. The independent specialists in our network are licensed healthcare providers subject to their own HIPAA obligations. Once a referral is made, the specialist’s office becomes the custodian of your protected health information (PHI).
We strongly encourage all patients and their representatives to review the privacy practices of their assigned specialist directly.
8. Cookies & Tracking Technologies
Our website uses the following types of cookies:
- Essential cookies: Required for the website to function properly (e.g., form submissions, session management). These cookies cannot be disabled.
